I moved into 1012 W Nickerson apartment in 5/2021. I moved out in 5/2022 due to become ill in the apartment. I have documentation from my doctor that I was ill due to "something" in my apartment. I took 7 days off of work.On 12/13/21, I made a maintenance request in regards to damp books and clothes, antiques swollen and the left side of the walk-in closet shelf falling/detaching from the wall. Under the area where the wood was attached to concrete wall was a black substance
