per letter: To Whom It May Concern, as my husband Timothy Hill reported per phone to the Seattle Department of Construction & Inspections on 08/26/16, I, Simone Hill, fell on the stairs leading down from apartment 403 (4th floor) to the 3rd floor (address: 5530 E Green Lake Way N, apartment #403, in Seattle WA 98103) on 8/25/2016 at approximately 5 p.m..
